This form specifies the documentation and information requested in support of an application for a transport license of radioactive substances from and/or to nuclear installations on Swiss territory. For the transport of nuclear materials and radioactive waste from and to nuclear installations according to the Swiss Nuclear Energy Act (NEA), an application must be submitted to the Swiss Federal Office of Energy (SFOE).

The application shall be submitted to ENSI in written form, including a side letter and the copies required. The use of this form is not mandatory if comprehensive information is provided otherwise.
